What Is Not Included
- In-hospital care
- Surgery/diagnostic procedures (except minor things such as minor lacerations, lancing boils, removing splinters, and other common primary care office services)
- Nursing home care
- Covered services for Medicare-eligible patients
- Prescribing or refilling narcotics for chronic conditions
Re-enrollment fee: $200 per person, regardless of age. (Non-refundable, applies only if you resign from the practice and then decide to return.)
Membership in Elite Primary Care is NOT health insurance. Members are encouraged to consult with an insurance professional to purchase health insurance coverage in line with their individual needs.
