What Is Included with Membership

5. Urgent care/sick visits

6. Sports physicals, school physicals, camp exams

7. Options for house calls, telehealth appointments, telephone, and text messaging

As appropriate, all at no extra cost

8. Direct access to YOUR DOCTOR 6:00AM - midnight 7 days a week.

Calls and messages received after midnight will be answered after 6:00AM the next day.

IF A CONDITION ARISES BETWEEN MIDNIGHT AND 6:00 AM THAT YOU FEEL IS AN EMERGENCY, CALL 911 OR GO DIRECTLY TO THE NEAREST HOSPITAL EMERGENCY DEPARTMENT.

Both for acute illness and for chronic conditions such as high blood pressure and high cholesterol. (This service will be available as soon as state registration is received.)

What Is Not Included

In-hospital care
Surgery/diagnostic procedures (except minor things such as minor lacerations, lancing boils, removing splinters, and other common primary care office services)
Nursing home care
Services for Medicare-eligible patients
Prescribing or refilling narcotics for chronic conditions

Re-enrollment fee: $200 per person, regardless of age. (Non-refundable, applies only if you resign from the practice and then decide to return.)

Membership in Elite Primary Care is NOT health insurance. Members are encouraged to consult with an insurance professional to purchase health insurance coverage in line with their individual needs.
